Booking your assessment

No, a referral by a physician is not required to access our services.

CareAxis offers some services free of charge to eligible patients through our programs. A referral from a physician is required to ascertain program eligibility. To learn more about our programs, click here.

The fastest way to book your assessment is online. Alternatively, you can call 1.844.MSK.AXIS (675-2947) to speak with one of our friendly Care Coordinators.

Your family doctor can refer you to CareAxis for an assessment (referral form). A referral is not required to access our services. However, a referral is required to ascertain eligibility for free services through our programs.

No, our broad network of surgeon-trained physiotherapists often allows for same-day appointment bookings, if requested.

Patients can easily specify the most convenient location when booking an assessment online.

Please change/cancel your assessment online (click here to log-in to your CareAxis account) no later than 24-hours prior to your assessment. Or, call 1.844.MSK.AXIS (675-2947) to speak to one of our friendly Care Coordinators.

Changes or cancellations to your scheduled assessment without 24-hours notice may be subject to a rescheduling or cancellation fee of up to 100%.

No shows will be subject to a 100% cancellation fee.

No, your existing diagnostic imaging (MRI, x-ray) / surgeon appointment will not be affected. Your CareAxis physiotherapist will provide you with advice as to whether diagnostic imaging / a surgeon consultation will be of benefit for your condition.

About your assessment

CareAxis physiotherapists are licensed to practise in Quebec. Our physiotherapists are rigorously trained by surgeons to expertly assess and manage your condition. CareAxis regularly reviews its physiotherapists and their clinics to ensure they meet our high standards. 

Your assessment will be 1 hour.

Your CareAxis physiotherapist requires this time to thoroughly assess your condition and to tailor a CareAxis treatment plan for your specific condition and personal requirements.

Your CareAxis physiotherapist will conduct a thorough medical history and physical examination. He/she will design a treatment plan that fits your specific condition and personal requirements.

Your treatment plan may include a referral to diagnostic imaging (MRI, x-ray) and/or a surgeon. You are under no obligation to continue with your treatment plan; however, it is recommended in order to fully benefit from CareAxis.

It is important that your CareAxis physiotherapist clearly understand your condition. If French or English is not your primary language, please consider bringing a friend or a family member to your appointment who can translate on your behalf.​ 

Please complete the online intake form that will be emailed to you after booking an assessment.

Remember to arrive to your assessment at least 15 minutes early to ensure that you get the most out of your time with your CareAxis physiotherapist.

CareAxis does not provide patients with medication prescriptions, but your CareAxis physiotherapist may recommend over the counter medications or additional therapy to support your treatment plan.

Your assessment indicates that you are a good candidate for diagnostic imaging  (MRI, x-ray) and/or surgery. You will be provided with access to diagnostic imaging and a spine surgeon as required.

You are under no obligation to accept diagnostic imaging, surgery, or any other medical intervention. 

The diagnostic imaging and surgeon consult is covered by Medicare.

Your family doctor will receive a consultation note that summarizes the results of your assessment, including any activity/work modifications, under the following conditions:

  1. You request CareAxis to share this information with your family doctor
  2. You are referred to CareAxis by your family doctor
  3. You are assessed to be a good candidate for diagnostic imaging and/or surgeon consultation. (CareAxis will first request your consent to share this information with your family doctor.)

Your CareAxis physiotherapist or a representative of CareAxis may contact your family doctor directly to discuss your assessment results, etc. if any of the above conditions apply.

Assessment price and reimbursement

A CareAxis assessment is $169.00 (our services are not subject to GST/PST/HST).

The cost of your assessment is reimbursable by private health insurance plans. Please contact your insurance provider for more information.

CareAxis offers some services free of charge to eligible patients through our funded programs. A referral from a physician is required to ascertain program eligibility. To learn more about our programs, click here.

Medicare does not currently cover the services of CareAxis.

Job Responsibilities:

As a member of CareAxis, the physiotherapist will be responsible for delivering patient assessment and education from his/her existing practice location. He/she will act as the primary point of care for consultation and assessment of referred patients and will identify signs and symptoms requiring diagnostic imaging and/or referral to a physician specialist. The physiotherapist will provide a defined program of care to the patient with an emphasis on education, self-management and health promotion strategies. He/she is responsible for documenting assessment results and treatment plans.

All CareAxis physiotherapists will be connected through a centralized patient management system. Please note that successful candidates must complete both practical and theoretical training by board-certified surgeons and other spine specialists to earn there CareAxis credentials. The training is free for selected candidates.

Skills and Qualifications:

  • At least 10 years of clinical experience managing patients with musculoskeletal conditions, in particular spine conditions, following completion of an accredited, entry-to-practice program in Physical Therapy, required.
  • Member in good standing of professional/legislative college at the provincial level with no history of disciplinary action, required.
  • Work out of a clinic that satisfies the high standards of CareAxis, required.
  • Advanced or extended practice certification (e.g., arthritis care), preferred.
  • Experience in academia, clinical research or public health, preferred.
